Job Description
Reporting to the Manager, Community Health and Wellbeing, the Indigenous-Focussed Public Health Nurse (PHN) will work with Indigenous partners to plan and adapt programs/services to meet the needs of urban Indigenous populations and help minimize barriers to them accessing public health programs/services. This role includes: identifying effective strategies/interventions that reduce health inequities and improve health outcomes in partnership with various government, school and Indigenous community organizations; supporting and modifying TPH programs and services in order to meet the unique needs of urban Indigenous populations; collaborating with various organizations, networks and groups to advocate for programs, services and resources that address/promote the social determinants of health.
Major Responsibilities:
The Indigenous focussed PHN also:
- Provides public health nursing service for individuals, families, groups, schools and the community according to the program specific policies, procedures, and operational plans of TPH, the Community Health Nursing Standards of Practice and the standards of the College of Nurses of Ontario
- Performs outreach and assesses school and community health needs and develops/maintains school and community partnerships to meet those needs, utilizing a consultative approach
- Participates in the planning, implementation and evaluation of health promotion programs, resources, and special projects in schools, with school boards and the community
- Assesses school/community health status, from a health promotion and behaviour change perspective, and assists the school/community in developing and achieving health improvement goals; makes referrals to TPH programs and community resources
- Facilitates partnerships and liaises with schools/school boards, community agencies, Coalitions and Indigenous organizations to address school and community health needs, access to TPH programs and services, and development of school/community action plans.
- Develops and facilitates presentations and other educational strategies to school and community groups
- Collaborates and consults with City staff, schools/school boards, Indigenous organizations, community agencies and coalitions to assess health needs and foster community engagement opportunities for marginalized/priority populations (includes collaborating with other PHNs within TPH and across Ontario).
- Provides training, guidance and support internally to staff (including para-professionals) and externally to school/community partners
- Presents and participates in professional conferences and workshops.
- Provides consultation and participates in the development and implementation of policies, operational plans and work plans
- Maintains records and necessary documentation according to the policies and procedures of TPH and the Standards of Practice set out by the College of Nurses of Ontario.
- Prepares reports and relevant correspondence
- Carries out special projects and necessary services in various schools and community locations
- Prepares and submits activity reports and data, as required
Key Qualifications:
Your application must describe your qualifications as they relate to:
- Baccalaureate Degree in Nursing and a valid licence from the College of Nurses of Ontario.
- Lived experience as an Indigenous person to Canada (First Nations, Inuit, Metis) or considerable experience in providing public health or community services to urban Indigenous clients/communities. (see special notes below*)
- Possession of a valid Ontario Driver's License Class "G" with regular access to a vehicle
You must also have:
- Ability to drive regularly within the Greater Toronto Area.
- Understanding of the health and social issues facing marginalized urban Indigenous communities in Toronto.
- Ability to incorporate Indigenous history, culture and traditional values into health service delivery and to provide services appropriate to clients and communities with diverse cultures, health beliefs and practices.
- Ability to conduct work in a non-judgmental way, using anti-oppressive practices and a harm reduction and trauma-informed approach.
- Knowledge of community development/mobilization and health promotion strategies.
- Knowledge of youth engagement principles.
- Excellent group facilitation skills.
- Ability to work flexible hours - overtime, shift work including evenings and weekends may be required.
- Ability to work effectively in interdisciplinary teams/groups and in community partnerships/networks.
- Excellent interpersonal skills with the ability to establish and maintain good public relations and positive working relationships.
- Excellent oral and written communication skills.
- Ability to support the Toronto Public Service values to ensure a culture that champions equity, diversity and respect.
Special Notes*
Lived experience as an Indigenous person in Canada (First Nations, Inuit, Metis) with in-depth knowledge of Indigenous cultural safety, as well as the history and present-day context of the First Nations, Métis and Inuit communities in Toronto and elsewhere is a requirement for this position.
Candidates with considerable experience working closely with Indigenous populations in providing public health or community services and an in-depth knowledge of Indigenous cultural safety, as well as knowledge of the history and present-day context of the First Nations, Métis and Inuit communities in Toronto and elsewhere will be considered for this position.
