St. Joseph's Health Centre, Unity Health Toronto, 30 bed Transitional Care and Reactivation Care Unit is located at Humber Church site (200 Church St.; near Jane Street south of the 401). The Reactivation Care Unit provides care for adult and geriatric patients who require therapy, reactivation and transition planning in order to transfer to the community and/or other supportive environments.

This unit focuses on reactivation and transition planning for patients who are waiting transfer to the community and/or other supportive environments. Working as a part of a dedicated, interprofessional team, you will deliver patient centered care designed with best practice principles. This team works closely with patients, families, and caregivers to provide reactivation therapy with a goal of enhancing and optimizing patient's functions and engagement in daily routines.
Monday-Sunday, Days and Weekends to support a 7-day model of care; occasional evening shift as needed
Position Summary
The Rehabilitation Assistant will provide high quality consultation, assessment and intervention for a variety of medicine patients within an Interprofessional Collaborative Practice philosophy. Being part of this team means being part of an exciting new initiative to help us support patients along their continuum of care and to shape how care is delivered.
Qualifications
- Post-secondary diploma from a recognized Rehabilitation Assistant program (e.g. combination OTA/PTA Program) required.
- Current Basic Life Support (BLS) certification required
- Knowledge/experience with neurologically impaired patients, medicine patients, wheelchairs & mobility aids preferred.
- Demonstrated competency in proper patient handling, transfers and safe mobilization
- Minimum 1 year experience working as a Rehabilitation Assistant in a rehab or acute care setting.
- Basic computer skills preferred.
- Must have excellent working knowledge of the English Language both verbal and written.
- Must have strong communication and interpersonal skills.
- Must be able to prioritize and organize duties and work well independently.
- Must be able to provide patient care in both group and individual settings.
- Must be able to provide support to multi-disciplinary team.
- Must be able to meet the physical requirements of bending, transferring, assisting and supporting patients.
- Satisfactory attendance record.
