OPPORTUNITY
We are hiring a Part Time Direct Care Worker to join our team.
This is a part-time position working a minimum of 2 shifts per week.
Facilitate the empowerment and normalization of individuals with a disability in a pre-vocational setting and at times in a recreational setting.
You will be assisting in the implementation of Personal Support Plans (PSP).
These are designed to promote personal welfare, independent living skills and social skills; provides assistance with all aspects of daily living.
THE ROLE
- Assist with the development & Implementation of PSP to enhance all members' overall life skills and quality of life.
- Required to be a role model and support members to learn strategies that will keep the community and the members safe keeping in line with treatment goals and strategies.
- Support members with complex behavioural needs; some members have involvement with Center for Addiction and Mental Health (CAMH) and the Ontario Review Board.
- Responsible for ensuring open communication of program and members needs by following set communication system (i.e., communication book, daily logs, seizure reports, etc.) as needed to provide consistent communication within the residence and the other programs
- Facilitates necessary communications with various services systems within and outside the organization, including relevant family members, keeping appropriate documentation, and advocates for members as required.
- Administers and records prescribed medications as per instruction from a medical practitioner and following the agencies' Medication Policies and Procedures.
- Assists members with personal care and hygiene, as necessary. This can include assistance with personal hygiene, feeding and dressing, position change, and the care, maintenance, and use of any equipment used for the support of the member.
- Ensures the health, safety, and welfare of the members by performing general house cleaning
- Assists in organizing leisure activities and ensuring community integration activities in accordance with their needs and interests
- Arranges and/or provides transportation to the day program, family visits and other places as needed
- Performs other related duties as assigned by the Program Supervisor
- Participation in meetings and trainings as required. `
WHAT YOU BRING
- In possession of a D.S.W, S.S.W, C.Y.W, Behavioral Science Technology or equivalent degree or diploma or actively enrolled in a diploma or degree program
- Attainment of a pharmacology certificate, safe management certificate, First Aid & CPR is required
- Valid G or G2 Driver's License required

Sunday from 2pm to 10pm and Monday from 8am-4pm
