Hours of work: Weekdays, with flexibility for weekends on clinic scheduling
Sunnybrook Health Sciences Centre has an exciting new opportunity for an occupational therapist assistant and physiotherapist assistant (OTA/PTA) to be part of a community-based health professional team that supports Home and Community Care Residential Accommodation (HCCRA) Programs in the Ontario Health Toronto Region. The goal of the new Centralized Community Health Professions Team (CCHPT) is to facilitate effective transitions to long-term care, supportive housing, home, or a rehabilitation facility. As part of the CCHPT, the OTA/PTA will work onsite at different HCCRA locations, implementing treatment plans assigned by OT and/or PT, for clients who are awaiting community transitions, to maintain and progress client’s everyday functioning toward meeting transition targets.
Summary Of Duties
- Perform a variety of appropriate therapeutic activities as directed by the supervision Occupational therapist and/or Physiotherapist. Therapeutic activities may include, but are not limited to; strengthening, range of motion, progress ADL’s and function, set-up of wheelchair and seating products, gait training, balance training, exercise classes, providing physical assistance to PT/OT during assessment/ treatment etc.
- Prepare, organize and conduct individual and group therapeutic activities with clients, as assigned under the direction of a registered Occupational Therapist and/or Physiotherapist
- Complete clinical documentation and daily workload measurement
- Monitor and maintain inventory of therapeutic supplies, assessment and resource forms
- Provide preventive maintenance to Occupational Therapy and Physiotherapy equipment
- Ensure the on-going integrity and function of therapeutic equipment and treatment areas
- Participate in special projects/ assignment related to program and service development and program improvement, including interprofesional staff education on safe moving, mobilizing patients and handling of therapy equipment
- Participate regularly in interprofessional care team, business and professional council meetings
- Conduct work in a safe manner; prevention, identification and reporting of safety issues and problems; taking corrective action, where possible; attendance at safety meetings / training / updates in accordance with the Hospital’s standards
- Directly supervise OTA/PTA students
- Other reasonable duties as assigned

Qualifications/Skills
- Diploma from an accredited college for the Occupational Therapist Assistant/Physiotherapist Assistant program required
- Min. 2 years of experience in community-based setting
- Previous experience with hospital-based setting preferred
- Previous experience working with clients with mental illness, addictions diagnoses and/ or responsive behaviours preferred
- Previous experience with minor wheelchair modifications and repairs and basic understanding of wheelchairs and wheelchair parts
- Basic knowledge of various types of exercise equipment, gait aids, wheelchairs and seating products preferred
- Proven ability to be responsible for carrying out assigned tasks with minimal supervision
- Excellent oral, written, and non-verbal communication skills required and work successfully in an interprofessional team environment
- Demonstrated understanding of rehabilitation principles, compensatory vs. remediation approaches
- Demonstrated professional judgment and interpersonal skills in collaborating with clients and their families, other health care team members and the community members
- Valid driver’s license preferred
