Project Coordinator - 6 months contractual position
The Project Coordinator, Integrated Programs plays a key role in supporting the successful planning and implementation of home healthcare initiatives and projects. This position collaborates closely with Business Development and Clinical Design management and leadership, Information Technology, Finance and operations teams to successfully facilitate work plans and activities involved in the planning, implementation, and evaluation of home health care programs and projects. The Project Coordinator, Integrated Programs is detail-oriented, highly self-motivated, committed, and hard-working with strong interpersonal and communication skills and works with multiple internal and external stakeholders in the healthcare industry.
Reporting to the Program Manager, Integrated Programs, the ideal candidate will possess strong project management skills, an understanding of healthcare, home care, hospitals and other government systems to successfully implement projects and other initiatives, as well as a proven ability to complete administrative and coordination tasks involving multidisciplinary teams. The Project Coordinator, Integrated Programs supports the completion of projects and programs on time, within scope, and supports work to meet financial goals and budgets.
The Project Coordinator will have the ability to work from home with minimal in-office days in a hybrid model.
KEY RESPONSIBILITIES/ACCOUNTABILITIES:
Project Management & Coordination (50%)
- Developing and monitoring project plans, including timelines and milestones.
- Ensuring communication and collaboration between internal teams and external partners.
- Preparing, reviewing, and organizing presentations and stakeholder documentation.
- Providing administrative support for meetings (e.g., working groups, steering committees) and taking minutes.
- Identifying, documenting, and managing risks, issues, and actions to support project success and escalate as needed.
Data Collection & Reporting (25%)
- Assisting with data analysis to support project objectives.
- Preparing and organizing reports that provide insight into project/program status and performance for operations, leadership, and funders.
Collaboration & Stakeholder Management (20%)
- Engaging with internal and external stakeholders to ensure smooth implementation of projects.
- Serving as a point of contact for project-related inquiries and providing guidance to staff.
- Coordinating with external vendors or consultants to meet project requirements.
Other (10%)
- Completing assignments that support continuous improvement efforts.
- Handling other requests from Transition Team leadership as needed.

What you will need to be successful:
- Project Management Certificate and/or PMI certification an asset
- 1 - 3 years of Project Coordination experience
- Healthcare experience preferred
- Able to work independently and as a team
- Proactive, results-oriented and strong problem-solving skills
- Ability to manage multiple concurrent projects
- Attention to detail and highly organized
- Excellent verbal and written communication skills
- Excellent time management and organizational skills
