We are actively recruiting for Remote Care Monitoring – Program Coordinator Permanent Full Time Days (9am-5pm)
The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional Care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 4 sites – the Main site (Church & Bloor St), Harbour Lights site and Specialized Care Centre located at (Victoria Park & McNicoll) and Remote Care Monitoring site (2 Overlea Blvd.). Position Summary:
The Manager - Remote Care Monitoring & Assess and Restore at the Salvation Army, Toronto Grace Health Centre is currently seeking a Program Coordinator ( in person at the 2 Overlea location) for Remote Care Monitoring (RCM)/RCM+. The RCM program provides safety devices (e.g. falls-alert/SOS pendant, medication dispensing device, etc.) to eligible clients. Safety alerts are monitored and responded to by the RCM 24/7 call centre, and devices are installed by the team’s field technicians, as well as via a hub-and-spoke model.
The Program Coordinator will also oversee RCM+, which is the clinical extension of the RCM program. RCM+ is comprised of the RCM technologies, as well as an interdisciplinary team and transitional PSW support, with the goal of supporting patients with their transition home post-discharge. The RCM/RCM+ program is currently providing service to 3500+ clients across the GTA and beyond, with the goal of continuing this growth via a spread and scale model. The Program Coordinator position is an excellent opportunity to become a part of a growing and innovative program.
Responsibilities:
- Support all day-to-day activities to ensure smooth operations, by providing support to the Manager of the RCM
- Serve as a team lead by actively providing support and guidance to RCM staff
- Facilitate and monitor implementation of program policies and practices (e.g. partnership pilot projects)
- Outreach: Partnership building, completing presentations for referring partners, dissemination of monthly newsletters, etc.
- Reporting & Evaluation: manage monthly survey data collection
- Support the RCM manager with overseeing partnership operations between Toronto Grace RCM and hospital partners
- Complete and consistently update tracking of all projects and engagement initiatives
- Work collaboratively with community partners
- Support program growth and development
- Assist with ongoing maintenance of SOP and employee training manuals
- Actively partake in intake process for RCM+ and oversee patient transitions
- Support with administrative tasks: agenda drafting, booking meetings/events, and planning where necessary.
- Support with marketing and communications (e.g. social media, ensuring program brochures/materials are up-to-date, etc.)
- Other related duties, as required
Qualifications and Education:
- Post-secondary degree required, (with education in a regulated health profession an asset (e.g. social work)
- At least 3 years of clinical experience required – experiences in both community health settings and hospital preferred
- Comprehensive knowledge of community resources and health care delivery systems in acute care and community settings
- Experience in a supervisory/team lead capacity is an asset
- Excellent interpersonal, communication, organizational and decision-making skills required.
- Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution
- Excellent time management and stress management skills are required.
- Computer literacy and basic proficiency is required with a demonstrated willingness to learn and adapt to new technology.
- Fluency in English (verbal and written) is required.
