Duties & Responsibilities:
- Assists clients in finding housing and supports them with the related services that are part of that process. This includes accompaniment to apartment viewings and advocating for the clients.
- Works in collaboration with co-workers on the housing team to offer housing support for housed clients.
- Determines the client's eligibility to receive financial assistance from AHT to cover their first or last month's rent and decides if a rent subsidy is needed.
- Mediates any issues or disputes between tenants and landlords and implements eviction prevention strategies and knowledge of the Landlord and Tenant Board.
- Creates a data base of known property owners and builds strong relationships with them to develop trusting tenancy partnerships.
- Connects with other agency housing providers to maximize housing connections and opportunities.
- Familiarizes themselves with the Employment and Social Services framework of both Ontario Works and Ontario Disability Support Program to know what dollar amounts are allocated to the client's basic needs and shelter and uses this information to determine the affordability of the housing being sought after.
- Documents all client encounters and case management details in the electronic medical record database.
- Completes bi-annual housing reports for the City of Toronto that reflects housing related data.
Qualifications & Experience:
- Bachelor of Social Work or two years of equivalent, relevant work experience in a social service field; especially with homeless clients
- Required to learn about Indigenous culture, history, traditions, medicines and healing approaches
- Comprehensive knowledge of client-centered interventions and community resources experience with case management and counseling
- Proficiency with electronic charting software
- Strong interpersonal, advocacy, collaboration and conflict resolution skills and the ability to work independently and as a member of a multidisciplinary team
- Valid Ontario driver’s license
