We are currently hiring part-time (approx. $20 - $23 per hour) Client Care Coordinator to work At Your Home Compassion Care.
Responsibilities
· Manage home care services with case intake, admission, and care coordination.
· Develop, implement, and evaluate individualized care plans tailored to each client's specific needs.
· Serve as a primary point of contact for clients, staff, and healthcare professionals to ensure clear communication and understanding of care needs.
· Conduct assessments of clients' needs to recommend appropriate services and interventions.
· Monitor and supervise care staff to ensure compliance with agency policies and quality standards.
· Facilitate regular follow-up visits and evaluations to track client progress and satisfaction with services provided.
· Address and resolve any issues or concerns raised by clients or caregivers promptly and effectively.
Requirements
· Post-secondary education in nursing, social work, or a related field.
· Minimum 2 years of recent experience in community health or a related field.
· Knowledge of the healthcare delivery system and community resources.
· Case management experience or recent home care management experience.
· Excellent communication, interpersonal, problem-solving, and decision-making skills.
· Ability to work independently and as part of a collaborative team.
· Familiarity with health care regulations, standards, and best practices.
· Additional language skills are an asset.
· A valid driver’s license and access to a reliable vehicle.
