Job Summary:
The Behaviour Support Resource Lead (BSRL) provides holistic behavioural health care and consultation centred on the residents and their care partners in accordance with guidelines and organizational standards within the Long Term Care Home. This position promotes the wellbeing of residents promoting their quality of life and providing leadership, support, education and training to other team members. As a dynamic member of the healthcare team and working within a holistic resident focused environment, the BSRL performs all duties in accordance with the Ukrainian Canadian Care Centre’s Mission, Vision and Values.
Key Responsibilities:
- Provides direct clinical support to the residents with or at risk for responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions.
- Along with other team members, provides specialized holistic best practice assessments and recommended behavioural intervention strategies to support the residents
- Develops comprehensive P.I.E.C.E.S assessments, as well as the implementation and direct clinical coaching on non-pharmacological approaches supporting the resident as needed.
- Within the respective scope of practice, conducts a thorough assessment and recommends, implements and evaluates therapeutic interventions and approaches with respect to the expressed responsive behaviours/personal expressions
- Collaborates with the residents, their families and the clinical/interdisciplinary team to create, share, implement and model individualized behavioural care plans.
- Ensures information and care plans are up to date and shared with residents and families
- Provides facilitation, coaching, mentoring and demonstration team leadership with the care team
- Participates in regular data collection and provincial BSO regular reporting requirement.
- Must be available to provide support to all shifts and on weekends, if required
Qualifications:
- Current registration with the respective clinical College in good standing. For example, Registered Nurse, Registered Practical Nurse, Social Worker, Occupational Therapist, Psychologist, etc.
- Minimum of 3 to 5 years of clinical experience in assessing and treating older adults with dementia, complex mental health, substance use and/or other neurological conditions
- Training in P.I.E.C.E.S. GPA, Montessori certification or willingness to take training
- Current certification in Psychiatric/Mental Health nursing, Gerontology or other relevant certificate
