Copernicus Lodge is a not-for-profit, charitable and ethno-specific organization made possible through the efforts of the Polish Canadian community and the vision of Father Michael Smith, OMI. Located in the heart of the Roncesvalles community in the heart of west-end Toronto, we provide a continuum of care in a predominantly Polish, Roman Catholic environment to meet the changing needs of our seniors. The Lodge is comprised of an Adult Day Program, 200 independent, affordable self-care apartments with a Supportive Housing program, and a 228 -bed Long Term Care home.
Job Summary
Copernicus Lodge is currently seeking a highly skilled and motivated LTC Nurse Practitioner (NP) to join our team to contribute to the quality improvement of resident care and services of the Home. This role is essential in managing and overseeing clinical policies, collaborating with physicians to deliver high-quality medical care, and leading specialized programs within our facility. The ideal candidate will have a strong background in long-term care, leadership experience, and a passion for improving resident care. This position will report directly to the CEO, with some reporting requirements to the Director of Operations.

Key responsibilities include, but are not limited to:
- Clinical Policy Management: Management of policy revisions and advisory to policy implementation to ensure best clinical practices and compliance with regulatory standards. Management of new policies, as necessary to reflect changes in legislation, standards of care, and facility needs.
- Medical Care Coordination: Collaborate with attending physicians and other healthcare primary providers in NP capacity to coordinate and deliver comprehensive medical care to residents. Facilitate communication among the care team to ensure continuity and consistency of care.
- Specialized Programs Oversight: Oversee and manage some specialized programs and services offered at Copernicus Lodge. Ensure these programs are effectively integrated into the overall care plan and meet the needs of our resident population.
- Clinical Leadership: Provide clinical leadership and mentorship to nursing staff and other healthcare professionals. Foster a supportive and collaborative environment to enhance team performance and resident outcomes.
- Quality Improvement: Evaluate clinical practices and outcomes to identify areas for improvement. Advisory to implementation of quality improvement initiatives and ensure that resident care practices meet or exceed established standards.
- Resident and Family Interaction: Engage with residents and their families to address concerns, provide education, and support care planning. Advocate for residents' needs and preferences in the care process.
- Regulatory Compliance: Ensure that all clinical practices comply with relevant regulations, standards, and accreditation requirements. Prepare for and participate in audits and inspections as needed.
- Training and Development: Develop and deliver training programs for staff on clinical practices, new policies, and specialized care techniques. Promote ongoing professional development and education for the care team.
Qualifications
- Education: Master’s degree in Nursing with a focus on Adult/Gerontological Nurse Practitioner or similar advanced practice degree.
- Certification: Board certification as a Nurse Practitioner (NP) in Long-Term Care or Gerontology is required.
- Experience: Minimum of 5 years of experience in long-term care settings, with at least 2 years in a leadership or supervisory role. Experience in managing clinical policies and specialized care programs is preferred.
- Skills: Strong leadership, organizational, and communication skills. Ability to work collaboratively with a multidisciplinary team and manage multiple priorities effectively.
- Licensure: Valid Nurse Practitioner license in Ontario, with current CPR certification
