The Social Worker will actively participate in the delivery of care and ongoing program development within the Adult Diabetes Management Program at Trillium Health Partners. This position provides service to a large outpatient clinic, providing services to patients living with diabetes across various stages of life including transitioning from pediatric to adult care, patients of childbearing age and all ages of adults. The program delivers service to patients with Type 1 diabetes, Type 2 diabetes, and diabetes in pregnancy through an interdisciplinary care approach. The focus of the role is not to provide ongoing or long term counselling or support but rather on ensuring patients are supported in system navigation, and are linked appropriately to community resources.
Position Summary
- Responsible and accountable for decisions and actions in the provision and management of patients with diabetes and associated co-morbidities within the scope of practice of Social Work as defined by OCSWSSW as a collaborative member of the interdisciplinary team
- Practitioner, consultant, advocate and educator to meet the complex needs of a patients with diabetes, their families and the interprofessional team members from the DMC
- Perform psychosocial assessments, case history information, short-term therapeutic intervention/counseling, to individuals living with diabetes and develop a goal oriented social work treatment plan and interventions; provide patient/family education and support to address psychosocial, emotional and financial needs of patients and families
- Provide referrals based on assessment
- Participate in program planning, resource development and evaluation
- Participate in fostering community linkages
- Participate in in-service programs, Best Practices projects, Social Work Professional Practice Group, and hospital committees as required
- Develops education materials to meet the needs of the clients
Key Qualifications
- M.S.W. with experience in supporting those living with chronic disease, often with multiple co-morbidities ie Renal disease, cardiovascular disease, depression and other mental health conditions etc.
- Membership in OASW and registration in OCSWSSW
- Clinical experience in a hospital setting, with outpatient hospital experience preferred.
- Ability to perform in a navigational role with the ability to link patients with ongoing/long term social work and mental health support services.
- Experience and understanding of relevant community resources and how to access these
- Understanding of physical demands and emotional challenges faced by patients living with diabetes across the lifespan
- Understanding of supports required by people living with diabetes and other comorbidities:
- Using a variety of teaching methods, incorporating the principles of adult education, motivational interviewing and CBT techniques within a short term case management model
- Knowledge of placement and the application process for LTC facilities including experience in completing capacity assessments
- Knowledge of all appropriate legislation is essential
- Knowledge of ODB and assistive living supports
- Excellent interpersonal, communication and organizational skills
- Reliable and flexible individual to work as a member of the interdisciplinary team
- Proficiency in computer, experience with electronic medical record and good keyboarding skills are essential
