Program Overview – Assisted Living Services and Home and Cluster Care The incumbent is responsible to the Supervisor for providing support services to clients as authorized by the Supervisor. Clients may either be living in their own home in the community or in an Assisted Living Service’s independent unit or Cluster Site. Job responsibilities are both generic to the PSW role and specific to the program area/site in which the PSW is scheduled to work. Work may be scheduled on an hourly or block of hours/shift basis.
What You Will Do
- Recognizes that she/he is assigned to work based on program needs, client preferences and PSW skill set as required.
- Recognizes that services are client-focused and delivered in a professional and culturally sensitive manner.
- Complies with all agency, unit and program policies and procedures as required and as communicated e.g., during orientation, at training, memos, meetings/minutes, voice mail hot line, reference manuals. This also applies to policies and procedures at other sites where the PSW must work closely with other partner agency staff.
- Provides service to clients relevant to the program and as authorized by the Supervisor and within the scope of practice as permitted by his/her certificate of training (e.g., PSW) and all WoodGreen/Unit and Program policies and procedures.
- Complies with all health and safety policies and procedures
- Able to provide service based on physical demands of the job which require physical strength, stamina and ability to undertake frequent light to moderate lifting (10-25 kgs), and frequent walking, bending, stretching and pulling and able to operate equipment (e.g., vacuum cleaner, mechanical lift).
- Reports any injury received at work to the Supervisor/On-call within 3 hours on the day of injury and/or any perceived injury that may occur because of work and if required, participates in an “early and safe return to work” program.
- Confirms his/her work schedule as required e.g. reports to office if not able to work to allow the office to schedule replacement staff, for community - obtain schedule by Friday prior to the week of service, responds to all office voice mail message whether PSW is able to do the work offered.
- Uses the Alayacare Mobile Application on their cell phone to clock in and out of their scheduled visits/shifts.
- Presents a professional image to clients and the community (dress code, identification), and conduct him/herself in a professional manner when engaged in all work-related duties, including travelling to or from a client’s home or carrying out business e.g., interactions with other health care professionals, medical escort (if applicable).
- Uses good communication and interpersonal skills to work effectively with program staff (e.g., Scheduler, Supervisor), co-workers, other service providers (e.g., Fife House, OH, nursing, social work, funder etc.) and clients as required. This includes promoting a positive workplace environment and team building with co-workers when working at sites where teamwork is vital e.g., Broadview Manor. This also applies to resolving or getting assistance with any conflicts when they arise.
- Follows all cluster routines and client service plans as indicated.
- Understands that the PSW may not report off duty until staff report to work. If no one reports to work, a co-worker is late, double booking occurs (2 staff report for the same shift), the PSW will call the office/On-call and wait for further instructions.
- Prepares meals as per the menu cycle and consults with clients to modify the menu (e.g., for birthdays, Christmas) on a weekly basis, and orders, checks and stores groceries as needed.
- Provides a verbal report at change of shift or break time with co-worker.
- Understands how to operate, trouble shoot and contact the Oncall 24-hour response team for emergencies as necessary.
- Documents accurately in the appropriate client and program records e.g., incident report, client file, attendance tracking, petty cash etc., in a timely and professional manner.
- Ensures there are sufficient groceries, housecleaning/program supplies and forms for use by completing the relevant form and communicating to the office and oncoming staff when supplies are low and/or equipment is not working.
- Responds to building service/maintenance issues appropriately and promptly e.g., submits work orders as required.
- Supports opportunities/activities for the clients to maximize and maintain their physical and mental abilities to the greatest extent possible e.g., exercise program, participation in care.
What You Bring to the Team
- Personal Support Worker Certificate and NACC Certificate (if applicable).
- Minimum one (1) year experience Providing personal care to seniors and people living with disabilities

- Comprehensive understanding of how to support clients in meeting their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (iADLs).
- Strong communication skills, encompassing verbal, written, listening, and reporting abilities, to ensure clear and effective interactions with clients and team members.
- Ability to work collaboratively and effectively within a team, fostering a supportive and cooperative environment.
- Flexibility and sound judgment in assessing and responding to clients’ diverse needs.
- Familiarity with behavioural management techniques and approaches to effectively assist clients in various situations.
- Knowledge of gerontology issues, healthcare systems, and social services applicable to the demographic served.
- Competence in food safety handling practices and maintaining a sanitary program environment.
- Physical capability to perform frequent light to moderate lifting, walking, bending, stretching, and pulling.
- First Aid and CPR certification is a valuable asset.
- Knowledge of using computers and technology-based solutions for service delivery, documentation, and accessing assessment data.
- Must be adaptable to transfer between different program locations as required.
- Ability to maintain professionalism and respect in all interactions with co-workers, clients, and community stakeholders.
- Skill in following directions and exercising sound judgment in decision-making.
- Committed to working collaboratively in a team-oriented environment with individuals from a variety of backgrounds and needs.
- Dedication to maintaining confidentiality and all aspects of client privacy.
