TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking for a Mental Health Case Worker to join our team in a permanent full-time role.
The Mental Health Case Worker will be part of the new Community Mental Health initiative that will provide mental health services, including case management and supportive counseling, to clients discharged after completing the 90-day follow-up program with Toronto Community Crisis Services (TCCS).
We are seeking a dedicated individual with a BSW or related degree and registration with the Ontario College of Social Workers. The ideal candidate will have 2-3 years of experience in crisis support, conflict mediation, and provide frontline services to seniors and newcomers, including those with mental health and substance use issues. Experience working with diverse populations, including LGBTQ+ and low-income communities, is essential. Strong skills in assessment, decision-making, health promotion, and community development are required. The candidate should be organized, team-oriented, and proficient in Microsoft 365 applications. Fluency in one of the community languages and knowledge of the catchment areas are assets.

DUTIES & RESPONSIBILITIES
· Conduct client assessments, identify support needs (e.g., mental health, physical health), and build rapport with community members and stakeholders.
· Collaborate with external service providers, advocate for client needs, and ensure proper referrals and follow-ups.
· Coordinate care, promote client inclusion in the community, and connect clients to case management, mental health support, and rehab services.
· Provide short-term support for clients with mental health, emotional, and substance use issues, and offer pre- and post-treatment assistance, including home visits.
· Accompany clients to appointments and participate in team meetings and program evaluations.
· Assist in the development, implementation, and evaluation of programs, maintaining work plans and tracking activities using evaluation tools.
· Support students, volunteers, and community members in program execution, and share reflections with relevant teams.
· Enter data into the database, maintain records, submit reports, and ensure confidentiality.
· Participate in outreach activities, promote TNO programs, and engage with community events, updating internal materials.
· Support community development initiatives, including advocacy, outreach, and local event participation.
Vaccination Requirement – You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.
Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.
