Right now we are looking for a Social Worker to work within our Adult Outpatient, Mental Health and Addictions Program. Working within a Stepped Care Model, this position provides a combination of individual and group based care. This program focuses on acute stabilization of symptoms, short term episodic care and discharge to the community.

Reporting Relationship: Manager of Mental Health
Schedule Requirements: Regular Day Shifts – some evenings may be required, subject to change based on needs of the program
Location: Wilson Site – Cross Site Coverage may be required – Must be willing to work in all areas of Mental Health & Addictions Program.
Hourly Rate Range: $44.67 - $57.90/hour

Position Responsibilities:
- Works collaboratively with the multidisciplinary team in the Outpatient program to complete screening and a detailed mental health assessment (including risk assessment) for all referred patients.
- Completes a psycho-social assessment and provides counselling with individuals, families and groups using a holistic approach and a variety of therapeutic modalities (e.g. educational, supportive, CBT/ DBT skills, solution-focused) to promote an optimum level of functioning.
- Delivers group education sessions to patients with various mental health diagnosis (e.g. PTSD, Depression, Anxiety)
- Develops and coordinates the discharge plan in conjunction with the treatment team including connection/referral to appropriate internal and/or community resources.
- Completes patient documentation, workload measurements and data requirements in accordance with Hospital, Ministry of Health and departmental standards.
- Technology literacy and usage skills preferred. Such as: video conferencing using MS Teams, OTN etc, and ability to use mobile technologies such as smartphones and tablets to connect patients to care.
- Acts as resource of discipline-specific expertise to other members of the treatment team.
- Works in a manner to ensure safety to oneself, patients, colleagues and the work environment.
- Participates in reflective practice as per identified College standards
- Other related duties as required
Qualifications:
- MSW obtained from an accredited School of Social Work.
- Current membership with OASW and OCSWSSW.
- Certification in the prevention and management of aggressive behaviour is an asset.
- Two (2) years’ experience in mental health and/or addiction facility or setting required
- Demonstrated skills in working with a broad range of patient groups and their families.
- Demonstrated skills in individual and group counselling.
- Certified in CBT (certification in the main theoretical principles and fundamental skills for CBT for mood and anxiety disorders)
- Experience in the delivery of CBT-I and CBT-P is an asset
- Understanding of and experience in the application of motivational Interviewing skills is required. Formal training in this area is a definite asset.
- Experience in applying DBT skills required.
- Understanding of and experience in the assessment and treatment for problem substance use, addiction and concurrent disorders is an asset.
- Understanding of and experience in Trauma Informed Care is required.
- Additional training or certification in trauma-related therapy is an asset (e.g EMDR).
- Excellent organizational skills with demonstrated flexibility and adaptability to work in a fast paced/changing environment
- Excellent interpersonal and communication skills and demonstrated ability to work autonomously as well as a part of the interdisciplinary team.
- Ability to respond rapidly and decisively to crisis situations.
- Excellent knowledge of community resources.
- Demonstrates cross cultural awareness and sensitivity.
- Good word processing skills including electronic medical record, MS Office required
- Experience , familiarity and comfort, with technology-assisted healthcare delivery, specifically video conferencing using various platforms and devices preferred
- Excellent attendance and a discipline free record is required
