Job Description
Reporting to the Manager, Business Operations, THP@home, the Project Coordinator will support a variety of complex projects, engaging internal and external stakeholders to implement hospital to home solutions. Working as part of a team, the successful candidate will play a key role in supporting the leadership of multi-disciplinary project teams through all phases of the project life cycle. The position also provides support to program business operations.
General Duties
- Working with the Clinical Manager and Manager, Business Operations the successful candidate will support he THP@home program with projects to expand clinical service offerings, and business operations functions
- Support project documentation including plans, meeting agendas and preparing presentations
- Support engagement and communication with stakeholders, and follow-up with action items
- Help to ensure all stakeholders are included in the planning of projects, and their input is incorporated
- Flag and report any issues or barriers associated with a project to Managers
- Liaise with enabling departments, such as IT/IS, Finance and Performance Analytics to advance program projects and tasks
- Assist with budgeting, service projections and proposal writing
- Support invoice validation, processing and tracking
- Assist with administration and maintenance of technology systems used by THP@home, such as electronic health records, patient education platforms, assessment platforms, and others as they are adopted
- Support data collection from various systems for monitoring, evaluation and reporting to THP leadership and Ontario Health
- Assist with Continuous Quality Improvement and work planning through regular engagement with patients/families and staff
- Maintain a summary of best-practice evidence for health system integration, and promote knowledge among team members
- Participate in the creation and delivery of innovative presentations and proposals on a wide variety of topics, for both internal and external stakeholders
- Perform cross-functional duties as assigned and/or requested; perform other duties consistent with the job classification as required
Skills & Qualifications
- Successful completion of either a university degree or college diploma in business, or health-related discipline
- Ideally 1-3 years of experience in healthcare environments such as hospitals, home care, government or private sector
- Excellent organizational, time management and project coordination skills with demonstrated ability to multitask and prioritize tasks effectively
- Superior analytical and problem solving abilities
- Effective interpersonal communications and ability to successfully build relationships with senior leaders, frontline clinicians and business partners
- Superior communication and interpersonal skills.
- Able to work effectively independently and with a team
