Position: ::Outreach Service Worker (Part time)
Hours of Work: 21 hours
Reports to: Health Promotion Coordinator

Position Summary:: Responsible for outreaching low-income, ethno-cultural communities and faith groups, and supporting the delivery of all health promotion activities and initiatives.
Responsibilities:: 1. Delivery of health promotion education outreach:

- Assists in the planning, implementation and evaluation of health promotion program activities

- Conducts workshops to promote healthy aging, active and independent living, and seniors’ safety in appropriate language using culturally sensitive approach as outlined in the project team training.
2. Performs other support duties:

- Completes and maintains records of project activities.

- Attends and participates in team meetings and training

- Collects program feedback regularly from clients, volunteers, and reports to the Health Promotion Coordinator any service issues or concerns.
Education::
-
- Diploma in Social Services or equivalent, preferably in with a focus in Gerontology
Experience & Skills::
- Work or volunteer experience with and knowledge of ethno-cultural and marginalized communities.
- Knowledge and experience with healthy active outreach, and seniors’ health issues and asset.
- Excellent verbal and written communication skills
- Bilingual in English and Cantonese (Read & Write both)
- Strong proficiency in Microsoft Office, specifically, Word and Excel
- Valid Driver’s License an asset
