Role Description:
The Palliative Education and Care for the Homeless (PEACH) Clinical Nurse Coordinator will provide mobile clinical care to a subset of PEACH clients with specialty nursing needs comprised of individuals with life limiting illnesses who are experiencing homelessness or are vulnerably housed. The Clinical Nurse Coordinator will work collaboratively within an interdisciplinary team and with external care providers and partnering services to provide direct or indirect safe palliative nursing care. The Clinical Nurse Coordinator will use their knowledge, skills, and judgement to provide nursing care to address complex medical and psychosocial needs and provide health and social service navigation while ensuring access to specialized care. This role reports to the Nurse Manager with day-to-day collaborative direction of the PEACH Program Lead.

Roles and Responsibilities:

Responsible for the overall management of nursing care for a caseload of approximately 25 palliative clients, maintaining the caseload, and outcomes of collaboratively developed nursing care plans with the PEACH interdisciplinary team.
Review and triage nursing referrals received from the PEACH interdisciplinary team, ensuring timely access to palliative nursing care within ICHA’s nursing practice standards, while collaborating with PEACH leadership and Home and Community Care nursing services to establish and prioritize PEACH nursing care priorities.
Organize, schedule, and attend nursing appointments both in person and virtually, including scheduled and emergent visits to perform comprehensive nursing clinical assessments in various patient settings, addressing both urgent and non-urgent situations.
Assess, monitor, and administer medications; evaluate and document patient responses and health status changes; collaborate with the PEACH interdisciplinary team to modify care plans for optimal patient outcomes.
Collaborate with the interdisciplinary team and external healthcare providers to develop and manage comprehensive nursing care plans and interventions in close collaboration with PEACH physicians and affiliated partners.
Experience or willingness to work with unregulated care providers (UCP) within a palliative care setting, including teaching, supervising, or assigning activities or procedures.
Act as a resource for partner and referring organizations, supporting and promoting education about the PEACH program and services.
Support smooth transitions of care for clients (examples include supporting clients transition from hospital to community or from shelters to housing).
Attends and actively participates in team meetings to represent nursing within the interdisciplinary team and affiliated partners, contributing to comprehensive nursing care plans for PEACH clients on the nursing caseload.
Participates in a monthly interdisciplinary meeting that may fall outside of operational hours. Overtime will be calculated with respect to our collective agreement.
Maintain high standards of client care and documentation in accordance with accepted College of Nurses standards of practice, the Regulated Health Professions Act, and ICHA policies.
Perform additional nursing duties as assigned by the Nurse Manager to support efficient and effective operations.

Qualifications:

Current registration as a Register Nurse or Registered Practice Nurse with the College of Nurses of Ontario in good standing with proof of professional liability protection/malpractice insurance.
Hold current CPR certification- Basic Life Support (BLS).
Certification in Hospice Palliative Care (CAPCE/LEAP/Fundamentals), or willingness to complete upon hire.
Ability to work both autonomously and collaboratively within a multidisciplinary team.
Minimum 2 yrs. of experience in palliative care and/or care for individuals with life-limiting illnesses (previous work in a community setting an asset).
Approaches care with a trauma-informed, equity-oriented perspective for clients experiencing homelessness, supporting them through the continuum of illness from diagnosis to end-of-life and bereavement.
Willing to work from a harm reduction, anti-oppression, and trauma informed framework; experience working with homeless or marginalized service users preferred.
Ability to travel to ICHA main office, various client settings across the GTA on a regular basis to support operational needs.
Strong interpersonal, organizational and administrative skills that include computer literacy and ability to work with EMR (OSCAR).
Police check/clearance documentation (i.e., vulnerable sector screen) required.
